Petersham, MA Petersham, MA Full-time Full-time From $39,520 a year From $39,520 a year The Family Supportive Housing Case Manager (CM) provides services to residents with substance use disorder and their children who live in GAAMHA Family Supportive Housing facilities.
These services include a full range of case management activities with a goal of developing self-sufficiency and sustained recovery for the individuals served.
Responsibilities and Duties:
The CM is responsible for the development and implementation of the individual family service plan and its elements with the residents and children.
The CM will establish parenting supports that foster family stability and healthy parent-child relationships.
The CM will assist the resident in utilizing support systems, appropriate resources, and available community resources to maintain recovery and wellness.
The CM will work with community partners to help support the active search for permanent housing.
The CM will work with community partners to help develop employment opportunities for residents and leverage pre-employment supports to remove barriers to employment for the client.
The CM ensures client has transportation to other needed services, such as interviews, community self-help meetings, medical appointments, court appearances, emergencies, and other community appointments.
The CM will administer urinalysis/breathalyzer tests to ensure compliance with program rules.
The CM will be responsible for all related recordkeeping and reports.
The CM will attend all mandatory meetings and trainings.
The CM will know and enforce the requirements and procedures for reporting suspected cases of child abuse and neglect in accordance with M.

The CM will adhere to applicable laws, regulations, and agency policies governing the disclosure of client identifying data and confidentiality.
The CM will network and represent GAAMHA/Family Supportive Housing in the community and will conduct themselves in a manner that reflects the values and professional standards of the agency.
Performs other duties as assigned.
Qualifications:
The candidate will have a bachelor's degree in Psychology, Social Work, or related field with 1 year of experience working with homeless families with substance use disorders.
A LADC II-, or 3-5-years' experience providing counseling and/or case management services to individuals and families with substance use disorders is accepted in place of a bachelor's degree.
The candidate must have a working knowledge of multiple pathways to recovery and other mutual aid support programs i.

, MAT/MOUD.
The applicant must have excellent interpersonal skills (verbal and written), ability to establish strong relationships at all levels, work individually within a distributed team, and be able to manage multiple priorities.
Be able to demonstrate cultural competence and carry out all duties in compliance with HIPAA and 42CFR Part 2.
Experience with electronic medical records, preferably KIPU is a plus.
The candidate must hold a valid driver's license.
Supervision Given:
No supervision given.
Supervision Received:
The Case Manager receives administrative supervision from the FSH Program Director and clinical supervision from the Clinical Supervisor.
Schedule:
Monday through Friday, 9 AM to 5 PM.
These hours are variable as some participants may require different hours.
